Requesting a Certificate of Insurance

Certificate of insurance (COI) requests for the new season will be accepted beginning 9/1/2025. Clubs must be active to request a COI.

Insurance Coverage

A benefit of a USA Volleyball membership is access to secondary medical insurance that can respond in cases of injury or illness not entirely covered by a member’s primary insurance.  This coverage is free of charge to members and coverage remains the same across USA Volleyball.

The insurance in place with USAV is one of the most comprehensive insurance policies for a junior volleyball organization.  Coverage is only in place during a USAV sanctioned event.  It covers bodily injury, property damage, general liability, and other areas.

What’s a “Sanctioned Event?”

Generally, CEVA sanctions “full-team” events, meaning team practices and tournaments that the office knows about.  Select fundraisers or team-building activities can also be sanctioned.  Open gyms are not sanctioned.

CEVA also sanctions camps, clinics, and leagues.  A separate sanctioning form for these events can be found on our website under the “Docs/Forms” tab.  Please contact our office if you have questions about obtaining sanctioning for an event.

How to Sanction a Tournament

Go to the “Docs/Forms” tab on the main menu, find the sanctioning application that fits your event, and submit it via e-mail to the CEVA office.  Submission of an application does not guarantee sanctioning will be granted.  Other events (e.g., tryouts) may have a different form or application that allows for sanctioning.
